ok i have a question. Whats the difference between this

I do not understand. The bottom one costs 40 and the top one costs 400. But they are the same right?.

You get 2 items per .. The only difference i can tell is the 10+1 on the top one and i can't figure out what the heck thats supposed to mean.

Sorry for my ignorance/sillyness . Just looking to buy some stuff and don't want to make a mistake.
